Bringing Health Home

Bringing Health Home "Bringing Health Home" is a program that provides Rancho Cucamonga residents with $50 worth (through a 1:1 match) of incentives to purchase healthy foods at farmers' markets. Through a generous grant from Kaiser Permanente and First Five San Bernardino, all Rancho Cucamonga parents with children 0-5 are automatically eligible for this incentive! Pregnant women are also eligible! If you are eligible and are currently not enrolled in the Program, please register by Rancho Cucamonga Lawyer filling out the form below.
